Payment Telehealth

Thank you for your request for our telehealth services. You should have received an email from us confirming that we have received your request. If you cannot find the email, please check your junk/spam folder.

IMPORTANT: Please make your payment at the bottom of this page. Once we have received your payment, we will review your request for our telehealth services and work on setting up an appointment for you.


– Billing address: The default country is set to the United States. Change the country if you need to. Do this by clicking on the flag. If you have a billing address in either Curaçao or Sint Maarten, please select “Netherlands Antilles” for your country.

– Mobile phone number: If you live in Sint Maarten, ignore the default “+599” and begin entering your phone number starting with “721” and the next four digits of your phone number. Example: If your phone number is 7215512345, you should enter “7215512”.

– Ship to billing address: Leave the box ticked.
